What is the Wellbeing Partnership?

The Wellbeing partnership is delivering health and social care services closer to home to the people who need it.

The Partnership is an integrated care team which commenced in the west of the island and is steadily expanding to cover the whole island. During 2023 the team will also establish a partnership for the east of the island.

The partnership incorporates government and third sector organisations working together to deliver a seamless, multi-agency, person-centred health and social care service that will deliver better outcomes for our population. It consists of locally-based health professionals who provide co-ordinated support for people to help them stay well in their own community, through a single point of contact.

Promoting equality and inclusion is at the heart of the integrated care model. 

Where is it based?

There are 3 wellbeing partnerships serving the western, northern and southern communities. These are based in the following locations:

The Eastern Wellbeing Partnership is due to be launched later in 2023.

Who is it for?

The Wellbeing Partnership is for anyone aged 18+ who is registered with their GP in these locations: Peel Group Practice, Southern Group Practice, Castletown Medical Centre, Ballasalla Medical Centre, Ramsey Group Practice.

Anyone registered at one of those GPs can drop in or self-refer. If you are a concerned relative, friend or neighbour, you can also enquire about the services or relevant assistance that might be available to someone you think is in need. Names must only be disclosed if the individual has given their consent.

What does it do?

Integrated care is about providing health and social care services where they are needed, giving service-users easy-to-access and continuous care in one location instead of ‘fragmented’ care across several locations.

We ensure that all people presenting with physical or mental health, social and wellbeing needs are assessed to ensure that they can be appropriately and safely supported in the community.

With a focus on health promotion, we optimise the health and wellbeing of the Island’s population by providing prompt assessment, advice, education and support for those aged 18 years and over who have experienced a recent deterioration in function or condition, and who may be at risk of admission to or prolonged stay in hospital or a Care Home.

We ensure community support services are delivered closer to home, ideally at home if possible. For Care Home residents, the Wellbeing Partnership teams will offer support to individuals and the staff looking after them there for a limited period of time.

We ensure that, as far as possible, you will only need to tell your story once. Red tape will be reduced, and care and support tailored to meet your needs.

If you or someone you know is in need of support, please feel free to call us, email us or drop in for help and guidance. You don’t need an appointment – you can simply turn up.

If you are a third sector or private organisation providing care, please contact us about your services and come along to our regular development meetings.

If you are a practitioner in Manx Care, please contact one of the Centre Leads to find out how you can get involved and join us at one of our regular development meetings.

Drop-in sessions and local appointments

We also hold regular drop-in sessions for a range of services, and offer locally based clinic space to statutory and third sector organisations so they can meet you closer to your home. Western Wellbeing Partnership holds regular drop-in sessions for social benefits, debt advice, hear2hear and others.

All our drop-in sessions run between 9:30am to 12 noon at the following locations in the North of the Island: 

  • Every Monday at Ramsey Town Hall from 9:30am to 12 noon
  • Every Wednesday at Ramsey Group Practice from 9:30am to 12 noon
  • Every Thursday at Jurby Health Centre from 9:30am to 12 noon
